Four Easy Ways To Order...Take Your Pick.
Order by E-mail, Fax, Phone or Snail Mail!

To order by e-mail use our on-line quotation form. Simply complete the requested information and you're on your way! We will contact you, if we need additional information such as paper colors or starting check numbers. No fax is needed for repeat orders of checks that have had no changes since your previous order. You may also e-mail us directly at bizforms@adelphia.com.
- To order by Fax call or e-mail us to obtain an order form. Our FAX # is 719-576-2790. Please include your name, your company name and your phone number when you request a faxed order form by e-mail. Also, include a void copy of your current bank check on all first time orders.
- To order by phone call 800-778-8577 or 719-576-2789 from 9AM-6PM MST. Having the same information available that is asked for on the order form will be helpful.
- To order by snail mail call to ask for an order form to be mailed to you. Fill in the order form and send it along with any artwork or special instructions. Please be sure to include your payment and mail your order to:

Order Confirmations & Shipments
We will e-mail or fax you a confirmation on your order and request that you return e-mail or return fax your acceptance of the order. This is the time to review all of the information entered on the order for accuracy and completeness. Your order will be processed and faxed to the specialty product manufacturer within 1 working day (Monday through Friday) after receipt of your order acceptance and acceptable form of payment. Our manufacturer normally ships specialty product orders within 5 to 10 working day. Some options such as special color matches and special finishes and coatings may take and additional 1-2 working days. Camera ready artwork and/or films are always forwarded to our manufacturer by Priority Mail at no additional charge, but you should allow an extra 2 to 3 working days, when custom artwork is required.
Shipping & Handling
Our specialty products are shipped by UPS Ground within the contiguous 48 states. We recommend requesting 2nd Day delivery to Alaska and Hawaii and 3rd Day delivery to foreign or overseas shipment destinations. Shipping is quoted and charged at our true cost. A $2 handling charge is added per order to cover the cost for faxing confirmations, for faxing your orders to the manufacturer and for submitting artwork to the manufacturer. There is no 2nd or 3rd handling charge when more than one specialty product is ordered at the same time. Payment of customs duties, value added taxes and other fees levied by government agencies on overseas orders are the responsibility of the customer.
Paying For Your Order
We accept Company Checks, Personal Checks, American Express, Discover, MasterCard and VISA as acceptable forms of payment on all orders. Sorry, but we can't accept COD orders. Payment by credit card assures you of the fastest possible delivery time. Check payments on overseas orders must be by certified check or international money order denominated in U.S. funds.
Repeat Orders
You will receive a Reminder Form with each order that includes your reorder job number. This number may be used to place an identical order in the future. Our specialty product manufacturer will keep your original artwork, special diecuts, etc. on file for at least 24 months, but they don't accept responsibility for any damage or loss to such artwork files. All other terms and conditions on repeat orders are indentical to terms and conditions on first time orders.
